High blood pressure, also known as hypertension, is the leading cause of heart disease and stroke. Lowering blood pressure should be easy, yet millions of people have uncontrolled blood pressure. We need new strategies to diagnose and treat hypertension so we can save lives and keep people healthy.

We are partnering with community organizations and businesses, including churches, barbershops, beauty salons, and more, to screen for hypertension and connect people with uncontrolled blood pressure into new care delivery models. Leaders in these organizations and businesses are trusted; their voices matter. They play an important role in the health and wellbeing of their community.

THE STUDY

We want to determine the most effective

care model for blood pressure control.

Is it home blood pressure monitoring with:

(1) Referral to Primary Care Provider
