Pressure Check!

Pressure Check is a clinical research study to address disparities in hypertension. The study uses a community based participatory research model to engage participants and test different models of care delivery for blood pressure control.

The study leverages the latest evidence-based practices and combines them into one model: a) community partnership to build trust and amplify the voices of community advocates around health; b) use of digital technologies to make care easy; c) integrated non-physician care teams that bring together nurses, pharmacists, and community health workers to deliver whole-person care; and d) stakeholder involvement to ensure the study findings are relevant to healthcare delivery networks, payors, and policymakers.

Pressure Check is located in 4 cities: New Haven, CT (Yale New Haven Health); Boston, MA (Massachusetts General/ Brigham); Norfolk, VA (Sentara Health); and Houston, TX (Houston Methodist); and is funded by the Patient Centered Outcomes Research Institute (PCORI).
